Confidentiality

Your information is strictly confidential and will be guarded closely by our staff as private. We will not release any information from your file without your written permission. Communication forms are available for patients to advise our staff how they would like to be contacted regarding appointments and test results.

Appointments

Please call our office at (919) 876-8225 with any questions or to schedule an appointment.  If you are a new patient to our practice, please bring your current insurance card and arrive at least 20 minutes prior to your scheduled appointment to allow time for completion of paperwork and to give our staff time to accurately add your information to our files. If you need to cancel or reschedule your appointment, we ask that you give us at least 24 hours’ notice so that we can offer this valuable time to another patient. If 24 hours’ notice is not given, a $40 fee may be charged.

Cancellations

Should you find it necessary to cancel your appointment, we ask that you give us at least 24 hours notice so that we can offer this valuable time to another patient.

Financial Policies

Our practice accepts Visa, Master Card, American Express and Discover for your convenience. We also accept Care Credit. As a courtesy, our office will file insurance for you.  Please have your current insurance ID card available at each visit so we can avoid filing errors.

Completion of Forms

A fee of $20.00 will be charged as patient responsibility for completion of forms and must be paid at the time you bring in the forms, including the following but not limited to: Disability, FMLA, and Leave of Absence, letters regarding flying and/or airline tickets and letters to employers
